Occupational Therapy Referrals
To make an Occupational Therapy referral either:
1. Phone Caroline Bowman - Occupational Therapist 0422 535 539
or,
2. Email information@ableliving.com.au. Please include your name, address, contact details and reason for referral and we will contact you to make an appointment.
or,
3. Download the referral form and email the completed form to information@ableliving.com.au
To be eligble to receive medicare rebates you must have a referral form for Allied Health Services from your General Practioner as part of the Chronis Disease Management (CDM) Program.
Private Health Funds may provide rebates for services. Please check with your individual fund. We provide services for DVA and NDIS clients.
